Join the Idaho Community Health Center Association (ICHCA) and the Northwest Regional Primary Care Association's (NWRPCA) CHW Institute for a free three-part webinar series — CHWs and Population Health 101-103. This series is designed to equip Community Health Workers and care teams with the knowledge and tools to strengthen care coordination, address population health management, and build sustainable CHW caseloads at your health center.

Attendees are encouraged to attend all three sessions, but each webinar can be attended independently.
Speaker:
This webinar series will be facilitated by trainers from NWRPCA's Community Health Worker Institute (CHWI). NWRPCA is a regional primary care association dedicated to supporting community health centers across the Northwest. The CHW Institute provides training, technical assistance, and resources to strengthen the CHW workforce and advance community health.
Audience:
This webinar series is ideal for Community Health Workers, care coordinators, case managers, clinical supervisors, and health center staff interested in expanding the impact of CHWs within their care teams.

Kelly Volkmann, MPH, Project Director of the Community Health Worker Institute for the Northwest Regional Primary Care Association. Ms. Volkmann is committed to advancing and advocating for Community Health Worker (CHW) programs in primary health care. Her work history includes providing direct care as a registered nurse in California and Oregon, and as a program manager for a robust team of CHWs at a Federally Qualified Health Center in Benton County, Oregon.
Ms. Volkmann’s experience working with CHWs, direct patient care, and public health has given her a deep appreciation of the unique qualities and value CHWs bring to healthcare systems, as well as the importance of building sustainable programs to support them.
