The ABCs of VBC: It Takes a Team

ICHCA is hosting a Value-Based Care Summit at the Grove Hotel in Boise, Idaho on September 19th & 20th. This summit will bring together subject matter experts from payers, providers, community-based partners, and various other organizations that impact VBC transformation in Idaho. The focus will be to discuss how to innovate, collaborate, and enhance quality, affordability, and accessibility to care for the patients our CHCs serve. 

Who should attend:

This training will be geared towards health center C-Suite and Quality Improvement leadership staff. 

Learning Objectives:

- Establish a foundation of understanding how value-based care transformation impacts health center operations and culture

- Create strong orientation to VBC for new and established staff members in CHC’s

- Define components for success in a value-based care environment

- Understand individual role and the team’s role in value-based care

$100 for members (must be logged in to access)*

Participant support is available to help reduce costs for your health center. ICHCA will provide support for up to three (3) participants. This does not limit the number of staff who may attend. Payment will be processed by ICHCA no more than 30 days following the conference. The check will be made payable to and mailed to your health center. In order to receive the support, a health center must:

- Attend the full event

- Submit an evaluation

Payment & Cancellation Policy 

Registration fee payments must be received prior to the training.  Payment may be made via credit card or check and mailed to ICHCA at 1087 W. River St., #160, Boise ID 83702.  Payment may be grouped for multiple participants. 

Cancellations must be made in writing. Cancellations made 30 days prior to the event will be given a full refund; cancellations made 14 days prior to the event will receive 50% refund; cancellations made after that will not be refunded. You are encouraged to send a representative in your place if you cannot attend. If you need to modify or cancel your registration contact Kaylin Faranda.

Ben Money

Senior Vice President, Public Health Priorities

National Association of Community Health Centers (NACHC)

Ben Money is the Senior Vice President, Public Health Priorities at the National Association of Community Health Centers (NACHC). Money’s 37-year career in health care began in community mental health and includes 11 years in local public health and 18 years with Community Health Centers. He holds a master’s degree in public health nutrition from the University of North Carolina Chapel Hill.  Money also served as President and Chief Executive Officer of the North Carolina Community Health Center Association (NCCHCA).  Money’s area of focus at NACHC is equipping health centers to address root causes of health disparities and overseeing executive leadership for the PRAPARE tool and strategy to address social drivers of health that influence the health of their patients and the community.  He also leads NACHC efforts to position health centers as essential components of state and local public health in post-pandemic system redesign.

Jenni Gudapati, MBA, RN

Value-Based Healthcare Program Director

Boise State University

Jenni Gudapati MBA, RN is the Director for Boise State University’s Value-Based Healthcare Program. In her two years in this role, she has co-chaired the Healthcare Transformation Council of Idaho’s Telehealth Task Force, serves on Medigold’s Board of Directors, and represents the university on Boise Chamber’s Healthcare Advisory Board. Jenni has served on both state and national boards and has been very active in collaborating with both state and federal healthcare and legislative leaders, presented at State and National Association Conferences on Telehealth, Innovative Healthcare Delivery and Payment Models and is currently working on her PhD in Public Policy and Administration.

Jenni has dedicated her career to patient advocacy, combining clinical nursing and passion for business development, to drive patient-outcome based healthcare delivery. Prior to coming to Boise State University, she served as Idaho’s Association for Home Care President, and on the National Association for Homecare and Hospice Government Affairs Committee which enabled her travel to Washington D.C. to meet with federal legislators on healthcare transformation. Her desire is to continue to use this knowledge and make a difference to better patient care and advance policy and regulation. Her years of experience as a ED, CCU, Home Health, nurse, and industry leader, has fostered the belief that technology and common sense healthcare can not only reduce costs but improve patient lives.

Nikki Burroughs

Provider Network Manager

Blue Cross of Idaho

Nikki Burroughs has been with Blue Cross of Idaho for three years in Provider Network Management and in the healthcare industry for a total of 8 years. In 2020, she drove the development and launch of a Rural Value Based Incentive program that broke new ground. Her passion for rural communities has supported leading the program to great success even during a unique time in our world’s history where Covid-19 could have easily hindered the project.  Prior to working at Blue Cross of Idaho, Nikki spent several years in the clinic setting as an Administrator. Nikki values her prior experience and exposure that provided firsthand understanding of what challenges lie within the four walls of a healthcare facility. Nikki has an extensive background in training and development, leading people for 30+ years. She now uses her innate ability to leverage these leadership skills to accomplish mutual goals between the payer and the Provider partners.  Nikki’s current role is to manage the contract arrangements and negotiations for your value-based reimbursements. Nikki monitors and audits the financial performance of your contracts to ensure that they are mutually beneficial and that we work together to reduce the rise of overall healthcare costs and improve quality for members. Nikki works to build and maintain relationships with the network working toward the same outcome. Coordination of provider partners with BCI internal staff to collaborate on identifying areas of opportunity to support members/patients with focus areas such Risk Management , Quality, and Cost Efficiency. Nikki can help assist in the resolution of escalated and /or complex provider concerns or barriers. Nikki leads quarterly Joint Operating Committee meetings with CHCNI/ Idaho Community Health Centers and is the main point of contact for your network as it relates to Value Based Incentives. Nikki is happy to be your partner and looks forward to continued success for Idaho Providers.

Angie Finley, MA, CRC, CPC, CPC-P

Manager of Provider Engagement for Risk Adjustment Programs

Blue Cross of Idaho

Angie Finley, MA, CRC, CPC, CPC-P, is the Manager of Provider Engagement for Risk Adjustment Programs at Blue Cross of Idaho and has over 15 years of experience in the healthcare industry, including medical assisting, medical coding and auditing, data analysis, claims, provider relations, and risk adjustment. In her current role her primary responsibility is the oversight and development of provider education and performance programs with a keen focus on clinical documentation integrity for accurate and compliant risk adjustment coding and billing. Angie has a passion for the important role that documentation plays in identifying complexities of the provider’s work, as well as training clinical and administrative support staff with the skills to recognize these intricacies and connecting the value of this work to value based care.  

J. Edward McEachern, MD

Executive Vice President and Chief Medical Officer

PacificSource

Dr. McEachern currently is the Executive Vice President and Chief Medical Officer for PacificSource’s markets in Oregon, Idaho, Washington, and Montana.  PacificSource is a physician-founded 88 year old not-for-profit health plan offering coverage in Medicare, Medicaid, and all commercial lines of business. 

A native of Nashville, Tennessee, Dr. McEachern earned an undergraduate degree in biology at Emory University.  As a Robert T. (Bobby) Jones Scholar at the University of St. Andrews in Scotland he earned a co-terminal master’s degree in biological systems theory and statistics.  He went to Case Western Reserve University for Medical School, a Pathology residency at the University of California in San Francisco, a second residency in General Internal Medicine and Fellowship in Health Services Research at the Cleveland Clinic.  He has practiced medicine in Georgia, Ohio, and Idaho

His academic career includes faculty positions at Vanderbilt’s center for Health Services, Emory University School of Medicine (Epidemiology and Biostatistics), Case Western Reserve University, and currently at the University of Utah where he is an associate professor of Orthopaedics in the School of medicine, and the Director of the Department’s Health Services Research program, and an Assistant Professor on faculty at the David Eccles School of Business at the University of Utah.  He is also a distinguished scholar in residence at Boise State University.

Chuck Hofmann, MD, MACP

Chuck Hofmann, MD, MACP practiced general internal medicine in rural Baker City, Oregon for over 35 years.  During that time he served as Mayor of Baker City for three terms, as President of the Oregon Medical Association, and as a member of Oregon’s Health Policy Board which was responsible for transforming Medicaid health care and the creation of Oregon’s Coordinated Care Organizations.  He has served as the Clinical Consultant for Eastern Oregon Coordinated Care Organization since its inception in 2013.  EOCCO serves 65,000 rural eastern Oregon residents with a delivery system that includes 7 Critical Access Hospitals, 5 rural Health Districts, 24 Rural Health Clinics, and 7 Federally Qualified Health Centers.  His responsibilities include clinical administration, delivery system transformation, and payment system innovation.  He is the recipient of many awards including the Oregon Health Sciences University’s Outstanding Alumni Award, the Oregon Medical Association’s Doctor-Citizen of the Year Award, the American College of Physicians Most Valued Teacher Award, and the Oregon Primary Care Association’s Outstanding Leadership Award.  He lives in Meridian, ID.

Nate Elkins

Supervisor AHJ-Fire Life Safety & Emergency Preparedness Program

Division of Licensing & Certification Bureau of Facility Standards

Currently serves as the AHJ for Fire Life Safety & Emergency Preparedness Program under the Department of Health and Welfare’s Division of Licensing & Certification that totals 867 facilities which includes 20,362 licensed beds.

He holds a Bachelor of Science Degree in Fire Administration and possess an Executive Fire Officer designation, Master Instructor, and Fire Inspector III-Plans Examiner. He also holds a certification in Emergency Management and Crisis Preparedness.

Martijn van Beek

Division of Medicaid Quality Director

Idaho Department of Health and Welfare

Martijn van Beek is the Division of Medicaid Quality Director at the Idaho Department of Health and Welfare. In his position he works on population health, Medicaid’s quality strategy, and he oversees the Healthy Connections Value Care team responsible for Medicaid’s value-based healthcare program. Martijn has a background in public health, data analytics and project management. Before joining Medicaid in 2022, Martijn worked on a wide variety of public health programs in the Netherlands and at the Idaho Department of Health and Welfare.

Sarah Schauman, MA

Vice President of Operations

Integrated Health Partners

Sarah Schauman is the Vice President of Operations at Integrated Health Partners, leading the team in the transition to risk as a clinically integrated network. She has more than 10 years of experience in population health management and assisting safety-net organizations in the transition to risk-based models. Sarah has focused her efforts in the Medicaid space on the utilization and implementation of state reform and waivers / Delivery System Reform Incentive Payment (DSRIP) programs, strategy development and implementation, and supporting the transition to value-based care in safety-net organizations. 

Prior to joining IHP, Sarah was a Manager with Premier’s Medicaid Transformation and Managed Medicaid service line where she worked nationally to provide expertise in moving organizations to value based care. Her consulting work included assessing organization readiness in value-based payment and care models, managing DSRIP programs in both Texas and New York, coordinating a new state Medicaid model in Idaho, completing community health needs assessments and implementation plans for non-for profit organizations to address community needs, and collaborating with state affairs to align consulting work with state and national reform efforts and trends. 

Sarah previously worked in hospital operations in Oncology for both Weill Cornell Medical College-New York Presbyterian Hospital and Memorial Sloan Kettering Cancer Center in New York City. During her tenure she oversaw departmental operations, clinical research, and staff management. 

Sarah received her Bachelor of Arts in Biology and Philosophy at Denison University and her Master of Arts from New York University in Bioethics.  Sarah enjoys spending time with her family and traveling and currently lives in Raleigh, North Carolina.

Emily Holzman

Director, Clinical Transformation

Azara DRVS

Emily Holzman is a Director of Clinical Transformation at Azara Healthcare. She earned her BA in International Relations from the University of British Columbia, after which she joined the Peace Corps for service in Guyana. Upon returning, she worked for the nonprofit Project Place in Boston, creating and delivering an employment training program for justice-involved men. She moved into healthcare IT when she joined Athenahealth’s Population Health team, where she worked primarily with large accountable care organizations. In 2018 Emily joined the team at Azara Healthcare to focus on working with primary care practices to leverage their data into meaningful improvements in patient outcomes, access, and equity. Emily became an NCQA Patient Centered Medical Home Certified Content Expert in 2018, is currently earning her Master’s in Public Health at the University of Massachusetts and is working toward her doula certification.
